As a Client Facilitator you will assist clients/carers to continue living independently in their home and community. You will be working with children, young people, adults, older adults and carers and/or those with mental health issues. Attributes we value include: • experience managing the care of people who have complex needs, dementia, chronic illness and disabilities • demonstrated ability to prioritise, plan and organise a busy workload • knowledge of community and health care resources and services networks • community development or project development experience • the ability to think ‘outside the box’, develop creative ideas and be solution-focused • excellent interpersonal skills and the ability to link people into a range of supports and services • initiative and the ability to contribute to a continuous improvement team culture • experience building carer resilience • experience in budget monitoring • commitment to respect and confidentiality • a current driver’s licence.
